Why did I receive two bills for my Emergency Room visit?You will receive two separate bills from us. One will be for facility charges related to emergency room testing and/or procedures, and the other will be a professional bill for the emergency physician's services, billed under PVD Medical Associates. The combined total of these two bills will match the "out of pocket responsibility" amount you agreed to at the time of service. If you have any questions about either bill, please contact us, as billing amounts can vary depending on your treatment, insurance, out-of-pocket benefits at the time of service. If your bill exceeds the agreed "out of pocket responsibility," please call our billing department at (210) 429-8313, Monday through Friday, from 8 AM to 5 PM for an adjustment.

What happens if insurance denies my claim?Emergency Room: Full Spectrum will appeal your claim on your behalf but at some point, we may need your assistance during the appeal process, we may contact you to simply “help” us in communicating with your insurance company, please respond in a timely manner.

I received an explanation of benefits (EOB) from my insurance company, will I have to pay this amount?
Once your claim is processed, your insurance company will send you an Explanation of Benefits (EOB). It is important to note that the amounts listed on the EOB are NOT A BILL. If you have any questions regarding billing, please reach out to our billing department at 210-626-8825, Monday through Friday, 8 AM to 5 PM. The EOB is sent by your insurance provider to outline the benefits they have agreed to pay based on your plan, as well as any amounts you may owe. These amounts may include what has been applied to your deductible or co-insurance. You may receive separate EOBs for the facility and the physician.
If you have any questions after receiving your EOB, our team is here to assist you. Our in-house billing experts can check the status of your claim to ensure it was processed correctly. The insurance claim process can take time, and appeals may be made if your insurance company initially denies coverage for certain procedures or costs that should be covered. Rest assured, you will never be billed for an amount higher than what was agreed upon at the time of service. How much will I have to pay for my Emergency Room visit?If you have insurance coverage: One of our Patient Care Coordinators will explain your Urgent Care benefits and collect your Copay (if any). An Urgent Care staff member will call you back to be seen by either one of our Physician Assistants or one of our Family Nurse Practitioners. Following your visit, a claim will be processed (which may take a few weeks) based on the services in which you received, if additional in-network insurance claim benefits are applied to your deductible or co-insurance, a bill will be sent to you. The type of insurance you have might influence the final amount.

Is Full Spectrum Emergency Room In-Network with my insurance provider?
- Full Spectrum Emergency Room is a freestanding emergency medical care facility.
- The facility charges rates comparable to a hospital emergency room and may charge a facility fee.
- The facility or a physician providing medical care at the facility may be an out-of-network provider for the patient’s health benefit plan provider network
- The physician providing medical care at the facility may bill separately from the facility for the medical care provided to a patient.
- Full Spectrum Emergeny Room is a network provider with Blue Cross Blue Shield of Texas (Traditional Indemnity Business, PPO/POS Network, Blue Essentials & Healthselect Network, and Blue Advantage HMO Network), MultiPlan, Curative Health Plans, Caprock Health Insurance.

How long will I have to wait before seeing a provider?At Full Spectrum Emergency Room we streamline the entire process of intake, treatment, and discharge. Our objective is to provide a non-rushed, comprehensive, quality experience by compassionate and professional staff. A provider will typically see you within a few minutes of arrival. It's our goal for most patients to be diagnosed, treated, and back home before most hospital emergency departments even begin treatment.

If I need to be transferred to a hospital, how is this coordinated?If a transfer is required to a higher level of care (i.e. a hospital), we will provide all of the coordination for this and you will be admitted directly into the hospital bypassing their Emergency Department. We have an ambulance transfer service to ensure your continuity of care.

What are observation suites?Observation suites are available in our Emergency Room only. Our observation suites allow us to keep a patient up to 23 hours who do not require direct hospital admission so we may monitor any ongoing conditions and ensure you are back on the road to recovery before being discharged.

Do you offer pediatric care for infants and small children in your Emergency Room?Absolutely! Full Spectrum Emergency Room is equipped and staffed to treat emergency patients of all ages, no matter . Our Emergency Medicine Board Certified Physicians have specialized training in pediatric trauma and can handle emergent conditions in infants and children of any age. With a compassionate, caring staff and specialized pediatric equipment you can rest assured that your little ones are in excellent hands. If sub-specialty care is required, we will arrange for direct hospital admission to the appropriate facility of your choice without having to wait in a hospital emergency department.

Who will be treating me in the Emergency Room?If your condition requires our Emergency Room services, you will be treated by a Emergency Medicine Board Certified Physician with specialized training and experience with trauma and emergent issues. The majority of our Physicians are veterans with decades of training to ensure you receive the best care possible.

How much will I have to pay for my Urgent Care visit?
You will be given a Medical Screening Evaluation (MSE), at no cost to you, by one of our Emergency Room Physicians to rule out any life threatening emergencies. During this evaluation, the emergency medicine physician and emergency room nursing staff will obtain your vitals and explain the necessary services that he/she deems necessary to continue with care or stabilize your symptoms. If at that time you choose not to move forward with the recommended services, you will be discharged at no cost to you.
If You Have Insurance: If you decide to continue with the recommended care, one of our Patient Care Coordinators will review your insurance benefits and provide an "Out-of-Pocket Responsibility" form. This form will explain any potential patient costs after your insurance claim is processed.
Once your insurance has been billed, you may be responsible for a portion of the costs, based on your policy. For example, if your deductible hasn’t been met or your co-insurance doesn’t cover certain procedures, you may owe up to the “out-of-pocket responsibility” amount presented to you at the time of service. This amount will be adjusted based on any payments already made.
If you are Uninsured: If you are uninsured and choose to continue with care, our Patient Care Coordinators will discuss your options using our “Self-Pay Schedule Fee” form. We offer discounted rates and flexible payment plans tailored to your financial situation. Full Spectrum Emergency Room is committed to transparent pricing and offers flexible self-pay options to ensure you can access the care you need.

Is Full Spectrum Urgent Care In-Network with my insurance provider?Full Spectrum Urgent Care is in-network with the following health plans: Tricare, Medicare, TriWest Healthcare Alliance, Curative Health Plans, Blue Cross Blue Shield, Aetna, Cigna, and Humana. However at this time, we are out-of-network with United Healthcare, and do not accept Medicaid, Oscar, or Superior Health Plans in our Urgent Care. Your urgent care benefits will apply to your visit when in-network.

Do you offer pediatric care for infants and small children in your Urgent Care?Our Urgent Care usually treats patients aged 6 months and older. If your child is under 6 months and experiencing a high fever, they will be seen in our Emergency Room. At Full Spectrum Emergency Urgent Care, we have streamlined the intake, treatment, and discharge process. Our goal is to offer a thorough, unrushed experience with care provided by our compassionate and professional staff.

Who will be treating me in the Urgent Care?If your condition requires Urgent Care services, one of our highly trained Physician Assistants or Nurse Practitioners will provide your treatment.

Will you send information about my visit to my doctor or specialist?With your consent, we can contact your doctor or specialist and share any necessary information to ensure a seamless continuation of your care. Simply inform us during your visit or call us afterward with the relevant details.

What do I need to bring with me at check-in?Please bring a copy of your driver's license, state ID, or military ID, along with proof of insurance (if applicable). Additionally, it is helpful to bring a list of any medications you are currently taking.

Do I need to make an appointment?Full Spectrum Emergency Room and Urgent Care does not require appointments at either of our locations for Urgent Care or Emergency Room services.

What's the difference between an Emergency Room and an Urgent Care?
Emergency Room: : Our Emergency Rooms are fully equipped with life-saving equipment, an on-site laboratory, and advanced imaging services, including CT scans, X-rays, and Ultrasound, to treat conditions that require immediate attention.
Urgent Care: Our Urgent Care centers offer on-site laboratory services and digital X-rays, and are capable of handling standard urgent care needs. We treat minor injuries and common illnesses, and also provide services such as school physicals, physical exams, employment screenings, inducing DOT Physicanls (appointment necessary), certain vaccines, diagnostic testing, and preventive care. -
What makes Full Spectrum ER & Urgent Care different?
Our facilities offer a hybrid model, providing both 24/7 Emergency Room services and Urgent Care services Monday through Saturday (9 AM to 9 PM), and Sunday (9 AM to 6 PM) for your convenience. With our flexible and cost-effective approach, you never have to worry about choosing the right care for your unscheduled medical needs. You’ll receive the appropriate level of care at a fair price.
The main difference you'll experience at our facilities compared to a traditional hospital emergency department is the enhanced convenience, compassion, and personalized care we provide. Texas House Bill 3276
- If we are not in-network with your particular health plan, Federal law requires insurance companies to process your ER visit at the in-network benefit level.

If any portion of the services is applied to your deductible or co-insurance, you may receive a bill for the remaining balance. The final amount you owe will depend on the specifics of your insurance plan.
If you are uninsured: Full Spectrum Urgent Care offers transparent self-pay rates, which will be clearly explained to you before any treatment. Our team will walk you through our pricing and discuss payment options that fit your financial situation. -

Why did I receive two bills for my Emergency Room visit?You will receive two separate bills for your emergency room visit. The second will be a professional services bill from the emergency physician, billed under either PVD Medical Associates or Hardy Oak Enterprises, depending on the location where you received care.
